The Role

We're hiring a reputed company Practitioner to serve as the primary clinical reputed company of contact for patients navigating their remote care reputed company.

In this role, you will reputed company clinical decision-making with a strong emphasis on reputed company-directed medical therapy for heart failure, as reputed company as adherence to evidence-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends. You will reputed company patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider to optimize treatment plans, reputed company medications, and determine the appropriate level of care. This role requires a high level of clinical autonomy, strong judgment, and reputed company collaboration with Registered Nurses and Patient reputed company teams to deliver reputed company, patient-centered, and reputed company remote care.

Licensure Requirements

What You'll Do

• Respond promptly to clinical escalations generated through remote patient monitoring, including reputed company vitals, symptoms, and laboratory results.

What You Need

• 5+ years experience treating patients with chronic diseases (CHF, Hypertension, T2D) either in an outpatient or inpatient setting at a high-performing medical center.
• 3+ years of cardiology experience with a reputed company on heart failure management and reputed company-directed medical therapy (GDMT).
• reputed company ANCC or AANP certification.
• Master’s Degree as a reputed company Practitioner with the willingness to expand state licensure as reputed company adds new markets and partners.

Compensation

The anticipated salary reputed company for this role is $125,000-$150,000 annually, which is based on role scope, level, and location. In reputed company to reputed company compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the reputed company. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
