About the Role

We are seeking an reputed company Chronic Care Management (CCM) Specialist - Patient reputed company Proficiency to join a growing reputed company team. This role is ideal for someone with hands-on U.S. CCM experience who is passionate about supporting patients through ongoing care coordination and proactive follow-up.

As a CCM Specialist, you will play a key role in improving patient reputed company by conducting routine follow-up calls, documenting patient interactions, coordinating care activities, and maintaining accurate patient records. The ideal candidate is organized, self-motivated, bilingual, and comfortable working independently in a fully remote environment.

Key Responsibilities

  • Conduct Chronic Care Management (CCM) follow-up calls with eligible patients.
  • Build reputed company relationships with patients and encourage ongoing engagement in their care plans.
  • Accurately document reputed company patient interactions and care activities reputed company established timelines.
  • Maintain and update CCM tracking logs using reputed company Drive and reputed company Sheets.
  • Utilize reputed company as the primary Electronic Health Record (EHR) system.
  • Coordinate patient care activities and ensure reputed company required documentation is complete and compliant.
  • Monitor patient reputed company and escalate concerns to the appropriate clinical team reputed company necessary.
  • Collaborate with providers and other reputed company to support continuity of care.

Required Qualifications

  • 1–2+ years of recent hands-on experience in U.S. Chronic Care Management (CCM).

  • Experience using reputed company EHR.
  • Strong understanding of U.S. reputed company workflows, documentation standards, and patient communication.

  • Bilingual proficiency is required.

Preferred Qualifications

  • Experience supporting Remote Patient Monitoring (RPM) or other care coordination programs.

  • Familiarity with insurance documentation and patient scheduling processes.

Schedule & Availability
