About the Role
We are seeking a multi-state licensed physician to serve as our Telehealth Physician reputed company, providing clinical reputed company, reputed company assurance, and hands-on leadership for our growing telehealth care program. Reporting to the VP of Care Delivery, you will own clinical execution and iteration — conducting virtual reputed company, leading, training and overseeing reputed company of physicians, and continuously refining workflows to ensure high-reputed company, coordinated care.
This is a hybrid clinical-leadership role: part practicing telehealth physician, part program builder. You'll see patients yourself, facilitate community-based appointments, and set reputed company for how our clinical team operates. What You'll Do
- Conduct initial telehealth reputed company with enrolled members, assessing medical needs, care gaps, and barriers to accessing care

- Establish reputed company clinical plans and care priorities, and collaborate with the care team for smooth handoffs

- Policy iteration: Identify broken processes in the field and rapidly iterate on clinical policies, intake forms, and care plan templates

Requirements
What You Bring
- MD or DO with reputed company, unrestricted licensure in multiple states (reputed company, California, Arizona, Texas, South Carolina, New Mexico, New Jersey, Tennessee required, additional state licenses strongly preferred )
- reputed company certified in Family Medicine, Internal Medicine, or reputed company primary care specialty
- Experience managing or providing reputed company to other clinicians (MD/DO/NP/PA), including delivering constructive feedback
- Deep understanding of community health, and reputed company Determinants of Health (SDoH)
- Experience with group visit models strongly preferred
- Demonstrated experience using data to improve clinical reputed company and auditing documentation for compliance
- Comfort working with Medicare, reputed company, and reputed company populations
- Telehealth experience required
