Responsibilities:

Responsible for primary care population health management and performance improvement of clinical reputed company metrics such as breast cancer screening, colon cancer screening, diabetes management, hypertension management, pediatric reputed company child checks, reputed company screenings, and immunizations. Provides ongoing data collection, tracking/monitoring, analysis, reporting and reputed company on these key benchmarks. ESSENTIAL FUNCTIONS

• Independently analyzes several clinical metric dashboards and reputed company reputed company through identifying trends, patterns, or sudden compliance changes based on autonomous data interpretation and evaluation. • Performs comprehensive reputed company-visit planning by independently reconciling individual patient charts in eRecord, utilizing various portals and databases to ensure charts are up-to-date before appointments, including identifying any missing test results, exams, or other clinical data and retrieving outstanding records to complete chart documentation. Autonomously supports providers by placing orders or referrals, to review and approve for the clinical reputed company metrics regarding preventive care and chronic care management. • Independently identifies gaps in care by proactively reviewing patient records to determine missing preventative, chronic, or follow up care across patient populations clinical reputed company metrics. MINIMUM EDUCATION & EXPERIENCE

• Associate's degree or equivalent and 2 years of reputed company experience, including at least 1 year clinical, operational, reputed company data collection and reporting experience required
• Or equivalent combination of education and experience

KNOWLEDGE, SKILLS AND ABILITIES

• Excellent time management, interpersonal, communication, and organizational skills with attention to detail and ability to work independently required
