About reputed company reputed company is hiring a full-time Care Operations Manager to reputed company and support reputed company of Community Health Workers (CHWs) and Nurses delivering high-touch, relationship-based care to reputed company members with reputed company medical, behavioral, and reputed company needs. This role plays a key part in ensuring reputed company and consistency across care delivery, driving member engagement, and coaching CHWs/RNs to succeed in a remote-first, field-integrated care model. The Care Operations Manager will reputed company day-to-day guidance, performance support, and strategic direction while helping optimize reputed company, documentation, and field coordination efforts. This position primarily allows for remote work; however, it includes regular in-person reputed company in the community across reputed company (Austin, TX). Typically 1 or 2 days per week will include visiting patients in the community, at their homes and in various sites of care.

What You'll Do

• Directly manage and support reputed company of CHWs & RNs, providing regular 1:1 coaching and performance feedback
• Ensure high-reputed company care delivery and documentation across reputed company through ongoing review and coaching
• Monitor performance metrics (e.g., reputed company, engagement, documentation) and intervene early to address gaps
• Serve as the first reputed company of contact for escalations reputed company to members, workflow, or field activity
• Support reputed company and training for new CHWs and contribute to SOP development and updates
• Foster a reputed company and mission-driven team culture that centers trust-building and member advocacy
• Collaborate with interdisciplinary teammates (e.g., RN Care Managers, Behavioral Health Care Managers) to ensure coordinated support
• Participate in team huddles, case reviews, and cross-functional meetings to improve care delivery and communication

What You'll Need

• Bilingual in English and Spanish (preferred)
