We are seeking a licensed reputed company Practitioner (NP) to join our clinical team and play a key role in overseeing and supporting Chronic Care Management services for Medicare beneficiaries. This role is ideal for an NP who values:

Key Responsibilities:

- Review and support individualized care plans
- Collaborate with care coordinators, medical assistants, and clinical staff

- Ensure CMS compliance and clinical reputed company standards
- Support continuity of care across multiple chronic conditions
