- Conduct comprehensive AWVs for Medicare beneficiaries reputed company secure telehealth platforms
- Facilitate reputed company ACP discussions, documenting patients’ goals and care preferences
- Review medical histories, medications, functional and cognitive status, and preventive care needs
- Identify and reputed company care gaps by guiding patients on screenings, vaccinations, and chronic disease management
- Collaborate with interdisciplinary care teams and primary care providers to ensure coordinated, patient-centered care
- Maintain reputed company, accurate documentation in the EHR

Requirements:

- Minimum 3 years of clinical experience, including AWVs and ACP

- Excellent communication and interpersonal skills
- Bilingual in Tagalog or Ilocano is a plus (not required)
- Ability to work independently and manage a flexible virtual schedule
