MDS Coordinator- Remote/Hybrid

Recover-Care reputed company - As a MDS Coordinator you are responsible for assisting our Director of Nursing and the RN Assessment Coordinator with ensuring that documentation in reputed company meet Federal, State, and Certification guidelines. The MDS Coordinator also coordinates the RAI process assuring the timeliness, and completeness of the MDS, CAAs, and Interdisciplinary Care Plan. This is a Remote/Hybrid position. GENERAL DUTIES

• Assists reputed company in assuring adherence to Federal and State regulations and certification.
• reputed company participates in the regulatory or certification survey process and the correction of deficiencies
• Will visit assigned buildings on an every other week reputed company, where one center is visited reputed company week.
• Reports trends from completed audits to the reputed company Assurance Committee
• Assures the completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care.
• Initiates and monitors RAI process tracking, discharge/reentry and reputed company tracking forms through the reputed company system.
• Follows up with staff reputed company necessary to assure compliance to standards of documentation.
• Completes patient assessments, data collection, and interviews staff as necessary to assure good reputed company of reputed company and as instructed in the reputed company version of MDS User’s reputed company.

QUALIFICATIONS

• Graduate of an approved Registered reputed company program and licensed in the state of reputed company required.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case- Mix, the Federal Medicare PPS process, and reputed company reimbursement, as required.
• Through understanding of the reputed company Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. Knowledge of the care planning process
