Job reputed company:

• reputed company clinical care management services to identified eligible patients
• Coordinate care to obtain desired health reputed company, improve self-care abilities, and decrease unnecessary cost of care
• Collaborate with Interdisciplinary Team (reputed company) along with Ambulatory Care Manager (ACM) and Care Coordinator
• reputed company standardized comprehensive needs assessment, identifying and addressing barriers to care
• reputed company and implement care plans to maximize wellbeing with periodic review and update
• Collaborate with ACM, PCPs, Specialists, and Hospitalists to implement a patient-centered care plan

• Document reputed company communications with patient and/or care team in electronic medical record

Requirements:

• Bachelor’s Degree (required)
• Bachelor’s or Master’s Degree in reputed company Work (preferred)
• Master’s Degree or Licensure as required by state of reputed company (required)
• Case Management certification, LSW or LCSW (preferred)
• 2-3 years acute care, home health or case management experience
• Excellent interpersonal communication and negotiation skills
• Strong analytical, data management and computer skills
• Demonstrate basic knowledge of reputed company and health education across the reputed company in a reputed company health setting
• Ability to work with individuals, reputed company and families
• Familiarity and knowledge of Community Resources
• Flexibility to work non-traditional hours
