- The Registered Dietitian/Health reputed company utilizes a reputed company process of assessment, planning, implementation and evaluation
- Engage, reputed company, and promote/influence members in reputed company reputed company to achieving and maintaining reputed company health status
- Conducts comprehensive evaluation of member’s health status and risk with use of clinical tools and information/data review
- Establishes a plan of care based on member’s health status/risks, education, and support needs
- Utilizes influencing/motivational interviewing skills to ensure maximum member engagement
- Provides coaching and support to reputed company the member to reputed company ongoing independent healthy lifestyle choices
- Develops and monitors established plans of care to meet the member’s goals
Requirements:
- Certified Diabetic Educator Registered Dietitian with licensure
- 3+ years of coaching experience required
- Require Bachelor’s Degree in health reputed company field (e.g., Dietetics, Health Education, Wellness, Health Promotion)
- Experience in wellness reputed company areas such as diabetes, weight management, stress management, nutrition, physical activity or health lifestyle behavior change
- Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in challenging situations.
