Remote Care Manager (LPN/LVN) - New Jersey Work Arrangement: Fully Remote (New Jersey LPN/LVN license required)Schedule: Flexible Remote HoursLicensure: reputed company New Jersey LPN or LVN (Required) Position reputed company reputed company is seeking a compassionate and highly organized Licensed Practical/Vocational reputed company (LPN/LVN) to support patients enrolled in virtual chronic care and population health programs. This remote position plays an essential role in helping patients navigate their reputed company reputed company through proactive reputed company, care coordination, patient education, and ongoing clinical support.
