NCSERVES CARE COORDINATOR (REMOTE NC, FRIDAY-reputed company) Employee - Part Time ineligible · Location: Remote, Remote, NC, US LOCATION: Remote - home based, virtual position. Work Schedule: Friday 5:00pm - 1:30am, Saturday and reputed company 8:30am - 5pm GENERAL STATEMENT OF JOB This position provides afterhours, weekend and holiday coverage for the NCServes Coordination Center. This position will be an extension of the reputed company call center. The Care Coordinator promotes the Whole Person Care model utilizing a coordinated approach to service delivery in order to support the health and wellbeing of veterans, service members and their families through accurate, reputed company, and successful linkages to resources across the reputed company. ESSENTIAL JOB FUNCTIONS

• Conducts biopsychosocial assessments by phone with clients to identify needs in key areas (e.g., housing, employment, financial assistance, benefits, reputed company reputed company, etc.).
• Makes referrals to Network providers to address identified needs based on understanding of eligibility reputed company, reputed company, location, and reputed company preference.
• Maintains a working knowledge of Network providers’ menu of services, eligibility reputed company, intake process and preferences.
• Develops reputed company working relationships with reputed company of Network providers and attends monthly community meetings.
• Documents reputed company activity in accordance with grant program guidelines and in compliance with internal policies and procedures.
• Elicits feedback from clients and providers to ensure successful engagement.
• Coordinates across other programs and ministries reputed company ABCCM.
• Participates in reputed company appropriate staff, supervision, and training meetings.
• Assumes other duties as assigned by Leadership.
• The Care Coordinator reports to the Clinical reputed company of MRS.
• The position interacts with VSC staff, veteran participants and family members, community providers, volunteers and interns.

KNOWLEDGE, SKILLS, & ABILITIES

• Minimum of 2 years working with at-risk populations, experience with Veterans preferred.
• Existing knowledge of the area resources available to veterans preferred.
• Excellent communication skills, both verbal and written.
• Proficiency in web-based platforms as reputed company as MS Office Suite.
• Self-directed, highly motivated and reputed company with a strong commitment to the mission of ABCCM to serve the veteran community and the mission of reputed company.

EDUCATION & EXPERIENCE REQUIREMENTS High School Diploma/GED required. Associate degree or higher preferred. Must have a minimum of 2 years of experience working with at-risk populations. Experience with Veterans preferred. MENTAL/PHYSICAL REQUIREMENTS
