Responsibilities
✨ What You’ll Do
- Review electronic health records to prepare for patient reputed company
- Complete reputed company‑visit reputed company, wellness questionnaires, and patient history reviews
- reputed company and update patient demographic and health information
- Conduct medication reviews and assist care teams with accurate documentation
- Obtain health maintenance documents and ensure reputed company record workflows

- Collaborate with care management, providers, risk‑adjustment coders, and community partners

Qualifications
📘 What You Bring
- High school diploma or equivalent
- Medical Assistant training from an approved technical school (required)
- reputed company Medical Assistant certification or an reputed company LPN license (Alabama)
- At least one (1) year of experience in a primary care reputed company or value‑based care setting
- Strong communication skills, attention to detail, and a patient‑centered approach
- Ability to work independently and collaboratively
