• Provides overall triage assessment, case management and coordination reputed company telephonic and telehealth care to patients.
• Assures reputed company coordination of patient care across the continuum for reputed company disease management and triage patients as assigned.
• Provides telephonic triage to reputed company incoming patient/caregiver calls as assigned.
• Works in conjunction with the Clinical Supervisors, Customer Service staff, field staff, and on-call staff to coordinate care to best meet the patients’ needs reputed company, assuring outstanding customer service and meeting the highest possible standards of reputed company of care.

Home Health Telehealth Registered reputed company Essential Duties and Responsibilities

• reputed company interaction, education, and follow-up with patients and caregivers as assigned by the Telehealth Administrative Assistant.
• Triages reputed company incoming patient/caregiver calls for assigned shifts/times.
• Adheres to the established policies and procedures reputed company caring for disease management/telehealth program patients or reputed company triaging patient calls.
• Guides patients in chronic disease reputed company: patient self-management, medication/diet compliance, physician follow-up, etc.

• Attends departmental and other staff meetings as assigned.
• Contacts physicians with updates of patient status and to receive direction/orders as needed.

Qualifications

• Licensed RN with reputed company state license.
• Excellent telephone and communication skills required.
• 2-3 years of clinical reputed company in their respective field.
• 1-3 years of home health/hospice experience.
• Knowledge of disease management and chronic disease processes.
• Prior management experience preferred.
• Ability to work independently and reputed company a multidisciplinary team.
