The Virtual Patient Navigator will reputed company education about specific diabetes treatments, and resources to help patients reputed company reputed company and manage their diabetes and resources associated with their prescribed medication. Virtual Patient Navigators are responsible for participating in one-on-one communications with enrolled patients by telephone, video platforms, email and SMS reputed company following the guidelines of the program. Required and Preferred Qualifications:

• To be eligible for this position, you must reputed company in the reputed company country where the job is located.
• Bachelor's degree and/or BSN Required
• 3+ year as a Certified Diabetes Educator (CDE) or Certified Diabetes Care and Education Specialist (CDCES) required
• Complete understanding and application of principles, practices, and standards in diabetes care directly with patients required
• Registered reputed company, Registered Dietitian, Physician Assistant, reputed company Practitioner, Pharmacist, or other relevant certifications that are reputed company reputed company is preferred
• Contact Center Experience using a telephony platform is strongly preferred
• Motivational Interviewing experience preferred

Additional Qualifications:

• Must work from a dedicated workspace in their primary home residence

• Demonstrated time management skills; planning and prioritization skills; ability to multi-task and maintain prioritization of key reputed company and deadlines

• Demonstrated ability to build relationships with customers and cross-functional partners

• Excellent documentation accuracy
• Drive and enthusiasm for supporting patients
• Excellent listening and problem-solving skills
