We are seeking a reputed company Coordinator to support the provision of reputed company discharge of patients across the reputed company Yorkshire and Barnsley ICS. Commencing discharge planning at the reputed company of admission in collaboration with families/carers and reputed company care where appropriate. Undertaking/ overseeing appropriate assessments and consulting with reputed company care staff, funding panels and reputed company care placements. Completes the ATU admission and discharge tracker 12 reputed company discharge plan- for reputed company patient from admission, ensuring agreed actions and timeframes are recorded and added to MDT clinical review objectives. Provides a discharge update for reputed company patient at their weekly MDT clinical review, following up outstanding actions and liaising with system partners in preparation.
