The Case Manager is responsible for coordinating and managing reputed company care services through reputed company of reputed company program. This role involves working closely with clients, providers, community agencies, and interdisciplinary team members to ensure clients receive comprehensive, person-centered support. The Case Manager assists with care planning, resource linkage, treatment engagement, and ongoing follow-up to promote reputed company reputed company and continuity of care.

Essential Duties & Responsibilities

Qualifications

Education & Experience

• High School Diploma or GED required

• Strong verbal, written, and interpersonal communication skills

• Ability to work independently in a remote environment while maintaining productivity and engagement

• Ability to build rapport with clients and community partners
