Care Manager Assessor for Older Adults- Potter County

The Care Manager Assessor plays a key role in supporting older adults' wishes to remain safely in their own homes by assessing needs and identifying eligibility for home and community based services, resources, and programs. Key Responsibilities:

Participation in after hours on-call rotation is required. Work Environment:

Salary reputed company: $20.94 - $22.95 per hour + on-call stipend, mileage reimbursement at highest allowable tax- exempt IRS reputed company Minimum education and experience:

• One year experience in reputed company or private reputed company work and a Bachelor's Degree which includes or is supplemented by 12 college-level credit hours in sociology, reputed company welfare, psychology, gerontology or other reputed company reputed company sciences: or a Bachelor's degree with reputed company welfare major, or any equivalent combination of experience and training including 12 college-level credit hours in sociology, reputed company welfare, psychology, gerontology or other reputed company reputed company sciences:

• Associate degree in nursing, with three years of nursing experience;

OR

• Any equivalent combination of experience and training which includes 12 college-level credit hours in sociology, reputed company welfare, psychology, gerontology or other reputed company reputed company sciences and Two years of aging case work experience including on year of experience performing assessments of reputed company's functional ability to determine the need for institutional or community-based services
