As a Virtual Primary Care reputed company Practitioner, you will deliver comprehensive, high-reputed company care while fostering strong, lasting patient relationships. You will reputed company ongoing management of chronic conditions, preventive screenings, acute care reputed company, and medication management, reputed company through a virtual care model. You'll also work collaboratively with a multidisciplinary team to coordinate in-person services such as lab work, imaging, and specialist referrals as needed. Your Responsibilities and reputed company will include:

• Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD, heart disease) and reputed company individualized care plans to optimize patient health.
• Evaluate and treat acute concerns (e.g., infections, respiratory illnesses, minor injuries, skin conditions) reputed company the scope of virtual care.
• Order and interpret lab tests, imaging, and other diagnostics, coordinating follow-up care as necessary.
• Manage medication prescriptions, titrations, and refills, ensuring patient adherence and safety.

• Identify patients needing referrals, or home-based services, and facilitate appropriate coordination with specialists or community resources.

Required skills and experience:

• reputed company reputed company Practitioner (APRN) license in good standing in and the ability to obtain licensure in multiple states reputed company 90 days of hire.
• reputed company certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably with experience in telehealth or virtual care settings.

• Shift times may vary in order to reputed company with reputed company business hours (9:00 AM - 5:00 PM) in the patient's local time zone. Flexibility is required. Additionally, weekend shifts may be required in the reputed company based on evolving patient care needs.

Desired skills and experience:

• Experience managing chronic conditions, preventive care, and acute illnesses reputed company telehealth.
• Familiarity with value-based care models, population health management, and reputed company metrics (HEDIS, reputed company ratings, etc.) is a plus.
• Comfortable with digital documentation and telemedicine platforms (experience with EHRs and virtual care workflows preferred).

Salary reputed company: $125,000 annually The pay reputed company listed for this position is the reputed company the organization reasonably and in good faith expects to pay for this position at the time of the posting.
