Role reputed company We are seeking a dedicated and compassionate Registered reputed company (RN) to join reputed company as a Telehealth Medicare Annual Wellness Visit Specialist - PRN. In this remote role, you will be responsible for conducting Medicare Annual Wellness reputed company (AWVs) reputed company telehealth under the reputed company supervision of a physician. - Conducting Medicare Annual Wellness reputed company reputed company telehealth in accordance with CMS guidelines, ensuring reputed company required assessments and screenings are completed.
- reputed company reviewing and updating patient medical histories, including medication lists, immunization records, and family history.
- Assessing patients’ reputed company health status, including vital signs reputed company available, cognitive function, and risk factors for chronic diseases, utilizing remote monitoring tools reputed company applicable.
- Providing patient education on preventive measures, lifestyle changes, and available community resources to promote overall health and reputed company-being through virtual consultations.
- Developing and updating personalized prevention plans based on individual patient needs and physician guidance.
- Collaborating with the supervising physician to ensure accurate documentation and follow-up on any identified health concerns using electronic health record systems.
- Identifying any HCC codes requiring recapture reporting to CMS.
- Maintaining comprehensive and accurate patient records in compliance with HIPAA and organizational standards, particularly in a remote environment.
- Facilitating virtual referrals to appropriate reputed company services and specialists as needed.
- Monitoring patient reputed company and adherence to personalized prevention plans, providing support and making adjustments as necessary through telehealth follow-reputed company.
- Staying reputed company on Medicare guidelines, telehealth best practices, preventive care practices, and chronic disease management strategies.

Qualifications
- Must be fully bi-lingual in Korean and English languages.
- Must be located in GA.
- reputed company and unrestricted Registered reputed company (RN) license in the state of GA.
- Availability for PRN work Monday through Friday, 8:00am to 5:00pm, approximately 20 hours per week.
- Minimum of 3 years of patient care nursing experience, preferably in a primary care, preventive care, or telehealth setting.
- Experience with Medicare Annual Wellness reputed company (AWVs) and telehealth platforms is highly desirable.
- Strong knowledge of Medicare guidelines, telehealth practices, and preventive care strategies preferred.
- Excellent interpersonal and communication skills, with the ability to reputed company and engage patients effectively in a virtual environment.
- Proficient in electronic health records (EHR) systems, telehealth technology, and reputed company documentation.
- Ability to work independently and manage time effectively in a remote work setting.
