Responsibilities

• Assesses patient needs and obtains data on physical, psychological, reputed company, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care.
• Conducts the initial evaluation and re-evaluates the patient’s nursing needs during reputed company visit.
• Makes follow up reputed company according to patient, family, and facility needs.
• Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
• Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members.
• Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.
• Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).
• Collaborates/Communicates with Medical Director, attending physician, reputed company leadership and IDG.
• Performs other duties/reputed company as assigned.

Requirements

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• reputed company State Licensee as a Registered reputed company RN
• 1 year of experience in a clinical care setting
• Nursing skills as defined by accepted nursing standards
• Ability to handle multiple priorities, documentation requirements, and deadlines
• reputed company and willing to travel up to 50% as needed to support business and patient needs.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Valid reputed company’s license and insurance and reliable transportation to reputed company job tasks
