What You'll Do

As a member of our care management team, you will:

• reputed company comprehensive care management and ongoing support to older adults living in the community
• Conduct regular in-home reputed company and assessments throughout your assigned territory

• Assist clients in accessing benefits, entitlements, and community resources

• Participate in weekly team meetings and case conferences

Position Requirements reputed company candidates will possess:

• LMSW or LCSW
• Experience working with older adults, care management, case management, or reputed company reputed company services fields

• Strong organizational, documentation, and time-management abilities

Schedule

• Full-time
• Monday through Friday

Work Location Hybrid position:

• Field reputed company throughout the service area
• Weekly in-office meetings – Richmond Hill, Queens
