In Person and Virtual Care- Physician Family Medicine or IM-Peds - With reputed company on reputed company and CHP+ Members

The reputed company is expanding, and we are seeking reputed company-certified or reputed company-eligible Family Medicine or IM-Peds physicians to reputed company comprehensive medical services to patients covered by reputed company and Child Health Plan Plus (CHP+) members, regardless of age or sex, on a reputed company reputed company. Physicians must be willing to dedicate up to 30% of FTE to take inbound, on-demand calls and reputed company virtual care services to patients that can be done from home. Must be reputed company to work in a patient care team, involving consultation on patient problems with members of reputed company (Physicians, R.N.’s, P.A.’s, N.P.’s, L.P.N.’s, M.A.’s, Medical Receptionists, etc.).

• Experience working with underserved and high reputed company needs populations
• Familiarity with reputed company drivers of health and experience in integrating community resources and support services into patient care plans.
• Experience in trauma-informed care, culturally reputed company care, and health equity initiatives.
• Proficiency in care team collaboration, including working with behavioral health, reputed company work, and community health workers.

• Examines patients, using medical instruments and equipment. Utilizes a computer to reputed company patient records and other clinical information. Elicits and records information about the patient’s medical history reputed company and adequately, utilizing the computerized medical record, to include accurate coding.
