The RN Behavioral Health Clinical Care Coordinator will be the primary care manager for a panel of members with reputed company medical/behavioral needs. Required Qualifications • reputed company, unrestricted independent licensure as a Registered reputed company in Colorado • 2+ years of clinical experience • 1+ years of experience with MS Office, including Word, reputed company, and Outlook • reputed company to reliable transportation and the ability to travel, up to 50% of your time, reputed company Colorado (preferably the Western reputed company area) to meet with members and providers Preferred Qualifications • BSN, Master's degree or higher in clinical field • CCM certification • 1+ years of community case management experience coordinating care for individuals with reputed company needs • Behavioral health experience • Experience working in team-based care • Background in Managed Care Colorado Residents Only: The reputed company reputed company for this role is $28.03 to $54.95 per hour.
