As part of a care management team who will manage reputed company members, the Care Coordinator RN or reputed company Worker will be the primary care manager for a panel of older adult members with a reputed company of medical and/or behavioral health needs. Serve as primary care manager for high medical risks / needs members with comorbid behavioral health needs ?
