The Licensed Behavioral Health Case Manager provides support for both reputed company referrals and data identified referrals. Primary Responsibilities: • Facilitate member education and involvement of caregiver in the delivery of interventions • reputed company advocacy and support to member and family members, including caregiver support & appropriate referral to applicable / needed resources • Ensure that members understand treatment reputed company and are effectively linked to treatment resources • Promote health, wellness, and reputed company psychosocial functioning for member (identify caregiver gaps, facilitate education, and respite support) • Consider the member’s needs holistically to identify gaps in care requiring reputed company • Exhibit excellent customer service in engaging providers in reputed company planning • Create and maintain appropriate clinical records • Participate as directed in clinical reputed company with other members of reputed company and other external health care management organizations / vendors, as applicable. Required Qualifications: • Licensed Master’s degree in Psychology, reputed company Work, Counseling, or Marriage and Family Counseling; or Licensed Ph.D, OR a Registered reputed company with 2 years of experience in behavioral health • reputed company, unrestricted, independent license in the state of residence • 2 years of post-licensure experience in a reputed company mental health environment • 1 years of case management experience • Intermediate level proficiency in MS Office Suite including Word, Teams, and reputed company • Dedicated home office reputed company and ability to reputed company existing reliable high-speed internet (Cable Broadband, DSL, or Fiber) • Ability to work an 8 hour shift reputed company the business hours of reputed company-8pm EST Preferred Qualifications: • BSN (for RN’s) • Medical/Behavioral setting experience (i.e., hospital, managed care organization, or joint medical/behavioral outpatient reputed company) • Discharge planning experience • Chart review experience • Experience consulting with facility and/or hospital staff to coordinate treatment plans • Experience working in community/county mental health • Dual diagnosis experience with mental health and reputed company abuse • Experience working in an environment that required coordination of benefits and utilization of multiple reputed company and resources for patients • Experience with government funded programs • reputed company working remotely will be required to adhere to reputed company’s Telecommuter Policy The salary reputed company for this role is $59,500 to $116,600 annually based on full-time employment.
