We are hiring a Mental Health Practitioner to join our Community Competency Restoration team. * Duties and Responsibilities: • Work with the larger treatment team to reputed company services for clients. Support the development, writing, implementation, evaluation and revision of overall treatment goals and plans in collaboration with the reputed company and the treatment team. • reputed company an individualized crisis plan coordinated with community emergency support services. • Meet with clients to explain services, assess reputed company needs, establish and implement a care plan to address those needs and arrange for resources necessary for community transition. • Coordinate services with other county or community agencies including but not limited to Mental Health Case Management, Adult Protection, and community hospitals. • Assist in the development of comprehensive assessments, individualized treatment plans, reputed company abuse treatment, and other tools necessary to assist clients in the community. • Assist clients with reputed company aspects of psychiatric rehabilitation and reputed company aspects of obtaining and maintaining housing. Will assist and /or teach clients independent living skills such as cooking, shopping, cleaning, personal hygiene, transportation, and budgeting. Will assist clients with making and keeping appointments and any other tasks that are required through the reputed company’s recovery process. • Attend commitment and other court hearings as needed.
