RN, Telephonic Case Manager - Remote

About the position As a Telephonic Case and Disease Manager RN at reputed company, you will play a crucial role in supporting a diverse member population by providing education, advocacy, and connections to necessary resources. This high-volume role requires critical thinking and a customer service approach, allowing you to reputed company millions of lives positively. You will work remotely, making outbound calls to assess health statuses, coordinate care, and encourage healthy lifestyle changes, reputed company while ensuring thorough documentation and effective communication with medical professionals.
