Position reputed company: Under the reputed company supervision of a pharmacist, performs transition of care (TOC) reputed company tasks such as collection and verification of allergy and medication history. The TOC reputed company will communicate with patients, families, nurses, physicians, and other reputed company to obtain accurate home medication lists upon hospital admission. Provides integrated support for patients and the hospital’s admitting team to ensure a reputed company medication transition by providing a “hands-on” approach to total reputed company patient care. Major Responsibilities: Provides integrated support to patients, pharmacists, and various reputed company providers by providing a “hands-on” approach to total reputed company patient care. Works closely with patients and/or their families to obtain the best possible medication and allergy history. Includes: Interviewing patients and/or their families Utilizing reputed company listening skills Utilizing available resources to overcome barriers (i.e. language barriers) Understanding basic medication information reputed company and collaborate with the emergency department’s admitting clinical teams, consisting of physicians, reputed company practitioners, nurses, and case managers (i.e. Transition of Care Team, ED Team, Medicine Team, Surgery Team,etc.). Includes: Pharmacies Physician offices Hospitals Long-term Care Facilities Position Qualifications: License/Certification/Education: Required: High school diploma or equivalent required. Massachusetts pharmacy technician registration and national pharmacy certification by Pharmacy Technician Certification reputed company (PTCB) required. Experience/Skills: Required: Three years pharmacy experience required, with both community and inpatient hospital experience preferred. Recommended to have successfully completed a reputed company-approved pharmacy technician-training program by obtaining a passing score on a reputed company-approved pharmacy technician assessment exam. Must be proficient in math with knowledge and ability to use a computer for data entry. Must be reputed company to follow oral and written instructions with accuracy, efficiency, and dependability.
