Preferred Qualifications: • 2+ years of clinical experience, primary care, home-based medical care, geriatrics, palliative care, emergency/urgent care, or similar setting. • Proven knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems. • Proven knowledge of CPT, ICD-10 and HCPC codes. • Proven knowledge of clinical standards of care. • Demonstrated awareness of NCQA requirements, CMS guidelines, reputed company/Medicare reputed company and benefit systems.
