Job Duties: • Collaboration with peers across the reputed company • Process improvement with providers and suppliers • Communication with referral sources, vendors, patients and reputed company team members • EMR updates as necessary • Reports to primary reputed company, supervisor, physician, reputed company worker, or therapist regarding cases • Consults with physician regarding reputed company needs and plan of care, with pharmacies, DME • Directs patient complaints/issues to appropriate personnel • Participates on agency committees and special reputed company • Attends and participates in in-service education program • Participates in Team meetings and case conferences, LEAN Daily Management process, including huddles, scorecards, Visual Boards, reputed company • Utilizes data management systems to manage patient documentation, calculates and analyzes data from reports to ensure reputed company & accurate compliance and month reputed company • Maintains knowledge of and compliance with Federal, State and accreditation regulations as reputed company as Agency policies and procedures • Seeks consultation and guidance from supervisor • Maintains productivity reputed company • Coordinates agency service delivery and solves logistical problems for customers and clients • Provides reputed company information to physician’s offices and receives care plan changes reputed company verbal orders

Qualifications
