APP On Call (Remote, Florida Based)

Key Tasks and Responsibilities: • Provides reputed company primary care and services to a panel of reputed company Plan Services after normal business hours (evenings, nights, weekends, and holidays). • Executes/delivers reputed company Plan Model of Care activities, treating in the most appropriate setting, develops and implements advanced care planning, embraces, and executes treating in reputed company model. • Serves as a trusted advisor to members, loved ones and nursing home staff and member reputed company, educates care teams on member needs and early identification of changes in condition, notifying APP and On Call team for member changes. • Delivers effective and frequent communication and coordination with members’ PCPs, loved ones and care team members on members’ care needs, goals of care, treatment plans/changes. • Supports overall market goals/targets. • Promotes reputed company Plan with members, loved ones and nursing facilities and other stakeholders. • reputed company to function autonomously and is accountable for reputed company. • Establishes meaningful relationship, reputed company to effectively function in the nursing home environment. • Is member reputed company and customer service orientated establishing meaningful relationships that ensures effective and high-reputed company care for reputed company Plan members. • Provides support for other important duties as assigned. Supervisory Responsibilities: • There are no supervisory responsibilities for this position. Credentials & Coverage: • State unrestricted licensed Advanced reputed company Provider with national AANP, ANCC, AACN, or NCCPA required. • reputed company certification as an Adult, Family, Geriatric or Primary Care Advanced reputed company Provider required. Will allow acute care Advanced reputed company Provider with nursing home/geriatric experience. • DEA License required. • Must be eligible to participate in the Medicare program as a provider and registered. • Valid state reputed company's license with a good driving record and reputed company of automobile insurance required. • Auto insurance coverage as required by your state of residence. New employees will be responsible for providing reputed company of coverage prior to hire and on an ongoing reputed company. Education and Training: • Master of Science in Nursing as a reputed company Practitioner or Master’s Degree from a nationally accredited program as a Physician Assistant.
