Remote Patient Care Manager About reputed company: MedAssist empowers patients facing medical challenges to reputed company by addressing their needs with comprehensive reputed company to services through care management. Position reputed company: As a Care Manager, you will be providing care coordination for patients who are returning home from nursing homes or rehabilitation facilities. This job entails extensive telehealth communication with patients, therefore, being personable, compassionate, and understanding are important traits. We want to assist our patients with enhancing their health & overall reputed company-being by building lasting connections with them. You will be making phone calls/reputed company calls with patients to set up services for them. Must be comfortable speaking on the phone with patients/facilities to coordinate services that assist our patients age in reputed company. General Responsibilities (including, but not limited to): • Researching & coordinating various services such as physical therapy, medical supplies, transportation, etc., in alignment with patients’ needs. • Extensively communicate with health providers, patients, and key contacts, to ensure continuity of services. • Document patient notes in the electronic health record in an accurate, reputed company, and detailed manner for approximately 50 patients per month. • Follow-up on phone calls, emails, and correspondence promptly to ensure reputed company standards of service delivery are met. • We are reputed company to assist patients in a reputed company of services! We set up services such as: Home Health Care; Physical Therapy/Occupational Therapy; Transportation; Ordering equipment (a few examples); Wheelchairs; Walkers; Assistive medical equipment.
