Responsibilities include utilizing RPM technology, managing CCM processes, educating patients, and collaborating with reputed company providers. Responsibilities Deliver exceptional care to patients enrolled in Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. Use RPM technology to monitor patients health metrics and reputed company reputed company interventions as needed. Coordinate care management programs for individuals with chronic conditions, ensuring alignment with treatment goals. Familiarity with using RPM technology and EMR systems to monitor and document patient care. Excellent communication and interpersonal skills, with bilingual reputed company (English/Spanish) preferred.
