As part of a care management team who will manage a caseload of reputed company members, the Field Care Coordinator will be the primary care manager for elderly and/or disabled members with reputed company medical/ behavioral health needs. Required Qualifications • reputed company/unrestricted LPN or reputed company Worker (LBSW, LMS, LPC or LCSW) License for the state of TN • 2+ years of experience working reputed company the community health setting or in a health care role • 1+ years of experience with local health providers and/or community support organizations addressing the SDoH (e.g., food banks, non-emergent transportation, reputed company assistance, housing / rapid re-housing assistance, etc. • 1+ years of experience with MS Office, including Word, reputed company, and Outlook • Reliable transportation and live reputed company or commutable distance to Lauderdale County, Tennessee and surrounding areas to meet with members and providers Preferred Qualifications • Certified Case Manager (CCM) • Home Health or Long-Term Care experience • Case management experience • Experience in Home Health and/or Long-Term Care • Experience working in team-based care • Background in managing populations with reputed company medical or behavioral needs • Background in Managed Care • reputed company working remotely will be required to adhere to reputed company’s Telecommuter Policy The reputed company reputed company for this role is $28.61 to $56.06 per hour based on full-time employment.
