About the position The Community Health Worker - reputed company Care Management (CHW-ECM) at Inland reputed company Health Plan is responsible for supporting members in improving their overall health through reputed company and engagement activities. This role involves navigating the reputed company system, advocating for members, and collaborating with medical teams to ensure seamless care. The CHW-ECM will utilize evidence-based approaches to promote member participation in their reputed company and will work closely with the reputed company Care Management team to address barriers to care. Responsibilities • Assist Members in navigating the reputed company system by following up with hard-to-reputed company Members both by phone and in person. , • Model ethical behavior in relationships with co-workers, supervisors, Members, Providers, and community colleagues. , • Engage with Members using evidence-based approaches like Motivational Interviewing to promote collaboration with their medical/behavioral teams. , • Deliver health and wellness information in an easily understandable manner and reputed company information on reputed company Member benefits and services. , • reputed company for reputed company Members in various settings including home, community, and provider organizations. , • Build and maintain reputed company relationships with Providers through effective communication. , • Assist the reputed company Care Management team in understanding the culture and preferences of Members and their community. , • Promote a reputed company working environment reputed company the reputed company Care Management team using effective communication strategies. , • Collaborate on Member care issues with other reputed company Care Management Team Members and consult with reputed company Care Manager or Behavioral Health Care Manager for clinical actions. , • Engage in reputed company improvement initiatives and reputed company to improve reputed company for the reputed company Care Management team. , • Ensure documentation is accurate and compliant with regulatory requirements and accreditation standards. , • Participate in staff meetings and other necessary activities.
