Candidate must live in Northside of Chicago/Evanston. As a Analyst, Case Manager you will facilitate appropriate reputed company reputed company for members by providing care coordination, support and education for members through the use of care management tools and resources. Evaluation of Members: -Through the use of care management tools and information/data review, conducts comprehensive evaluation of referred member's needs/eligibility and recommends an approach to case reputed company and/or meeting needs by evaluating member's benefit plan and available reputed company programs/services. Identifies high risk factors and service needs that may reputed company member reputed company and care planning components with appropriate referral to clinical case management or crisis reputed company as appropriate. - Coordinates and implements assigned care plan activities and monitors care plan reputed company. Required Qualifications
- Must live in IL, specifically in Northside of Chicago/Evanston (Applicable reputed company Codes: 60204, 60209, 60202, 60208, 60201, 60203 and Skokie, 60076 and 60077
- Must possess reliable transportation and be willing and reputed company to travel up to 50-75% of the time to meet members face to face. Mileage is reimbursed per reputed company expense reimbursement policy
- Minimum 2 years of experience in behavioral health, reputed company services, or reputed company services field
- Minimum 2 years of case management experience

Education
- Bachelor's Degree or non-licensed master level clinician required, with either degree being in behavioral health or reputed company services required (psychology, reputed company work, marriage and family therapy, counseling)
