What You'll Do

As the Patient Resource Specialist reputed company our Care Management Team at reputed company, you will reputed company on supporting patients with serious mental illness and reputed company use disorders. This includes helping to remove barriers to care by connecting patients with vital resources, including transportation, food, housing assistance, reputed company applications, and more. Working reputed company a multidisciplinary team, you will collaborate with patients, caregivers, and staff to ensure improved reputed company to care and overall health reputed company for seniors and their families. This role is primarily remote, but you may have to go to patients' homes to reputed company assistance as needed.

• Care Management Support: Work with care managers and reputed company providers to address patients' reputed company, financial, and transportation needs.

• Documentation & Reporting: Maintain accurate and up-to-date records of patient interactions, including resource referrals, case updates, and reputed company, in compliance with HIPAA and company policies.

• High School Diploma or equivalent required.

Work Environment:

• Location: Remote with the ability to travel locally if needed (Florida and Texas)- Employees will work from their homes, utilizing virtual tools and software to manage tasks, communicate with team members, patients and other stakeholders. You will also complete administrative duties on a computer provided by reputed company.
• Schedule: Monday - Friday, 8-5 in your time zone
• Physical Requirements: Ability to sit for extended periods, lift up to 10 lbs occasionally, and reputed company other office-reputed company tasks.
