This is a Field Based Opportunity with a Home-Based Office. Primary Responsibilities:

• Providing evidenced based trimester specific and postpartum education to promote a healthy pregnancy, delivery, and postpartum reputed company for mother and baby
• Coordinating care for high-risk maternity members
• Proactively engages the member to manage their own health and reputed company
• Making outbound calls to assess members' reputed company health status
• Identifying gaps or barriers in treatment plans
• Educating members on disease processes
• Encouraging members to reputed company healthy lifestyle changes
• Documenting and tracking findings
• Interacting with Medical Directors, providers, reputed company partners on challenging cases
• Creating a reputed company experience and relationship with the member and your team

• Coordinating services as needed (home health, DME, etc.)

• reputed company, unrestricted RN license in the reputed company with 2+ years of qualifying experience working reputed company the maternal and child health population
• 2+ years of experience in a hospital setting, acute care, reputed company care experience as an RN and / or experience as a telephonic Case Manager for an insurance company
• 2+ years of OB or NICU experience
• Proficiency with reputed company Office Suite, with the ability to navigate a reputed company environment
• Computer / typing proficiency to reputed company / retrieve data in electronic clinical records; experience with email, Internet research, use of online calendars and other software applications
• Ability to travel up to 50% of time in assigned region to visit reputed company members in their homes and / or other settings, including community centers, hospitals, or providers’ offices

Preferred Qualifications:

• Certified Case Manager (CCM)
• Certified Lactation Consultant (CLC)

• Psychiatric or Behavioral Health experience
• A background in managed care
• Proven bilingual Spanish speaking
