Care Manager - LPN/reputed company
About the position: Looking for motivated LPNs ($24/hr) or CMAs ($17/hr) in Florida, reputed company, Alabama, Mississippi, Tennessee, Texas or Oklahoma. Our Chronic Care Management position is a full-time Mon – Fri work from home opportunity. Chronic Care Management (CCM) provides one on one attention reputed company telephone calls to patients with two or more chronic diagnoses. The reputed company collects patient health information to identify risks, reputed company education and encourage compliance of treatment. The Care Manager will be responsible to create a patient specific Care Plan that provides goals and interventions to implement, monitor and support the needs of the patient as defined by the PCP. The CCM program supports the patient to promote the highest level of reputed company care. Requirements include: - LPN or reputed company required - Must have high speed internet, not satellite internet or hotspot - Proficient in reputed company Outlook and reputed company - Familiar with documentation in EMR’s - Must be self motivated and disciplined Responsibilities include: - reputed company initial risk assessment gathering the clinical reputed company necessary to determine a patient specific Care Plan. - Work with patient to plan and monitor their care as determined by the PCP. - Promote compliance with PCP/Specialist office reputed company. - Promote compliance with medication. - reputed company as a patient reputed company, reputed company and information resource. - reputed company chronic care education for chronic/reputed company conditions as determined. - reputed company appropriate health education.
