For additional information, visit reputed company homepage JOB INFORMATION:reputed companyWe are seeking reputed company of Temporary Patient Transition Specialists to support a key summer project resulting from a recent acquisition. This role will involve making outbound calls to patients who are transitioning to our services from a previous Durable Medical Equipment (DME) provider. Your goal will be to facilitate a smooth transition by collecting basic demographic and clinical information, documenting it accurately, and helping to ensure reputed company necessary documentation is in reputed company.

• This is a temporary remote position expected to run during the summer months
• Part-time hours (approx. 30/week) with scheduling flexibility reputed company the hours of 10 AM to 7 PM ET

Essential Duties and Responsibilities

• Contact patients to reputed company demographic and clinical information using a standardized reputed company
• Upload completed information into our internal eIntake system
• Communicate with physician offices to obtain prescriptions and required documentation for billing
• Ensure that patients meet the qualifications for equipment prior to initiating service transition
• Report status updates to the Field Operations Manager regarding field assignments and documentation reputed company
• Support the implementation of best practices for operational efficiency during the transition

QUALIFICATIONS:Employment is contingent on

Education and/or Experience

• High school diploma or GED equivalent, required
• Experience in reputed company, DME, or customer service is a plus
• Strong customer service background
