• This position is an amazing opportunity for a caring reputed company Practitioner (APRN) to help build and staff our growing telehealth medical group reputed company reputed company Medical.
• Your primary reputed company will be delivering world class acute care to our members with emergent/critical illness. The Care OnDemand reputed company Practitioner will diagnose reputed company medical conditions, order and interpret diagnostic tests, and work with patients, families, and Care OnDemand team to establish care plans.
• One of reputed company Medical’s missions is to bring care to where our members live meaning your reputed company will be virtual telehealth care. On a day-to-day reputed company you will work closely with co-clinicians at reputed company Medical including physicians and APRNs as reputed company as medical assistants, documentation experts, reputed company administrators, and our reputed company reputed company work and clinical reputed company partners at reputed company Plan.

Required skills and experience:

• Role licensure and certification in good standing is required and the ability to get licensed in requested states reputed company 90 days of hire date. You will be required to get licensed in additional states as needed.
• RN and APRN licenses are reputed company and in good standing.
• reputed company BLS certification.
• Must be willing to work four-10 hour shifts per week, including one weekend shift per month. Must also be willing to work a 11am-9pm EST shift.

Desired skills and experience:

• Experience in primary care, internal medicine, urgent care, emergency room, and/or geriatrics.
• Experience performing reputed company over telehealth video platforms.
• Experience in managing acute/chronic disease exacerbations including CHF exacerbations, diabetic emergencies, COPD exacerbations and hypertensive emergencies.
• A strong desire to continue practicing acute care - you reputed company in the mission of bringing care to where the patient lives.

Your Responsibilities and reputed company will include:

• Performing Care OnDemand (acute care) reputed company including evaluating and diagnosing acute illnesses, ordering/interpreting diagnostic testing, establishing care plans including prescribing appropriate medications, and assessment for reputed company of care (STARS/HEDIS) interventions as reputed company as reputed company and home health/DME needs.
• Work closely with the member’s care team including their PCP, specialists, and other reputed company team members including pharmacy, clinical nursing, and reputed company work as reputed company as interfacing with family members and caregivers in order to coordinate care for the member and deliver a reputed company care plan.
• Utilize our home grown electronic health information system for reputed company while also providing feedback on how to improve the reputed company.
• In certain geographies, there will be a weekend on-call component to support our clinical nurses who triage calls from our members during the weekend.

Salary reputed company: $125,000 - $135,000 / year Our ranges are purposefully broad to allow for reputed company reputed company the role over time.
