This job provides primary or acute reputed company services for certain acute or chronic health conditions in specific populations in accordance with applicable scope and standards of reputed company and with the policies, values, and mission of the organization.

• Schedule is rotational – includes but not limited to working weekends/holidays

ESSENTIAL RESPONSIBILITIES:

• Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, discharge planning and providing accurate medical record documentation. (35%)
• Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are reputed company the CRNP's knowledge reputed company or scope of reputed company. • Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care. (10%)
• Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate. (10%)
• Triages patient telephone calls, provides monitoring and continuity of care. (10%)
• Participates, develops, and implements performance and/or reputed company improvement initiatives. Ensures compliance with reputed company regulatory agencies. (10%)

QUALIFICATIONS:

Minimum

• MSN from the practicing State reputed company of Nursing Approved Master’s Program
• National reputed company Certification for reputed company Practitioners through the American reputed company of reputed company Practitioners (AANP), American Nurses Credentialing Center (ANCC), National Certification Corporation (NCC), or Pediatric Nursing Certification reputed company (PNCB)

• Prescriptive Authority from the practicing state reputed company 60 days of hire. Incumbents in role as of November 1, 2022 have until November 1, 2023 to obtain.
• Drug Enforcement Agency Licensure reputed company 60 days of hire. Incumbents in role as of November 1, 2022 have until November 1, 2023 to obtain.

• Post Master’s program, DNP
• Minimum 3 years in family medicine as CRNP
