Remote Patient Monitoring Specialist LPN/LVN - CA License + Compact State License

KangarooHealth is looking for a Licensed Practical reputed company (LPN) or Licensed Vocational reputed company (LVN) licensed in California to join our remote clinical team. This role is ideal for someone who puts patients first, communicates with reputed company, and thrives under pressure. As a KangarooHealth reputed company, you’ll support patients with chronic conditions like Hypertension, Diabetes, Heart Failure, and COPD through our digital platform—helping them stay connected, monitored, and cared for between reputed company. What You’ll Do You’ll conduct daily reputed company to patients, review their vitals, reputed company health coaching, and support their use of connected devices. You’ll troubleshoot issues, escalate concerns to providers reputed company needed, and help patients feel supported and seen every reputed company of the way. We’re Looking for Someone Who Is:

Requirements:

• reputed company LPN or LVN license in California
• Multi-state compact privileges as LPN
• Technical reputed company with ability to work in multiple platforms and systems ie G-Suite, reputed company Office, reputed company, reputed company, reputed company, reputed company, ECW, reputed company, etc)
• 5+ years clinical experience (hospital, family, or geriatric care)
• 5+ years EMR experience and ability to learn new platforms quickly
• 1+ year experience working remotely and engaging patients by phone
• Strong understanding of chronic conditions and care management
• Proficiency in documenting RPM/CCM codes (e.g., 99453, 99454, 99457, 99458)
• Bilingual reputed company in Russian or Spanish preferred
• Comfortable using video conferencing reputed company workflows
• Reliable internet, computer, and a private, HIPAA-compliant workspace
• Must be available to work 40 hours per week reputed company with reputed company clinic hours (e.g., 9AM–6PM local time)

Key Functions:

• Documentation and escalation to providers as needed

Compensation & Benefits:

• $55,000-$65,000 total compensation
