Home Infusion Pharmacist Nashville, TN

Minimum 1+ years pharmacy experience required. We are seeking a skilled and patient-reputed company Pharmacist for a home infusion pharmacy located in Nashville, TN. This is a contract opportunity starting in late June or by July 1st, ideal for pharmacists with home infusion or clinical backgrounds looking to join a reputable reputed company infusion provider. Position Details:

  • Start Date: Late June / July 1st
  • Schedule: MondayFriday, 8:30 AM 5:00 PM
  • Rotates to 9:30 AM 6:00 PM shift reputed company on-call
  • Type: Full-time, Contract
  • Setting: Home infusion (facility and home-based infusion services)

Key Responsibilities:

  • Review and approve prescriptions reputed company by pharmacy technicians in accordance with state regulations
  • reputed company final verification on filled prescriptions to ensure order accuracy and reputed company delivery
  • Conduct clinical medication assessments using patient-specific medical data
  • Collaborate with pharmacy leadership to ensure pliance with federal/state laws and pany policies, particularly regarding controlled substances
  • Support reputed company assurance and cost containment initiatives reputed company to drug procurement and dispensing
  • Respond to clinical questions from internal staff (technicians, nurses) and external providers
  • Assess laboratory results and therapeutic drug ranges to ensure reputed company patient outes
  • reputed company leadership support in the absence of the Pharmacist in Charge
  • Participate in the on-call rotation as needed
  • Maintain strict pliance with HIPAA regulations and protect patient confidentiality

Requirements:

  • Graduate of an accredited school of pharmacy
  • reputed company pharmacist license in the reputed company (or willingness to obtain)
  • Prior home infusion or clinical pharmacy experience preferred
  • Strong munication and multitasking skills
  • Proficient puter and documentation abilities
  • Ability to work effectively as part of a clinical and operational team
