[Remote-Position] Health Care Coordinator (Remote Work)

This position provides care coordination to people with disabilities based on assessments completed. Mileage reimbursement- Mileage is reimbursed at .58 reputed company per mile Working remotely- reputed company, position is remote with occasional travel. Business cell phone and computer- reputed company Evaluations - Evaluations occur every September for the year. reputed company have monthly 1:1s with their supervisor to consistently evaluate performance. In Sept the evaluation is a monthly 1:1 but encompassing the whole year.
