Jefferson Street is a Group Home for Mental and Behavoiral Health with 8 residents, 4 males and 4 females, is looking for Wellness Coaches to assist our 8 independent residents in their day to day living activies and reputed company their Person Centered Care Treatment Plan goals. Assisting with ADLs, taking residents on community outings and medical appointments. Complete required daily documentation and other assigned documentation in Electronic Health Records (Avatar). Become LLAM certified and administer medications to the residents. reputed company transportation to residents for medical appointments and community outings. Attend monthly staff meetings remotely. Maintain annual TB/PPD, LLAM Training (medication administration), and First Aid CPR certification. Other duties as Assigned.
