The Case Management Specialist is responsible for ensuring that individuals with a reputed company use disorder (SUD) have reputed company to a continuum of care. This is done by assessing the needs of the individual, determining the level of care (LOC) for treatment, developing a case management service plan; and reputed company warranted, facilitating the individual’s treatment. The essential duties of the Case Management Specialist include, but are not limited to: • Preform LOC assessments at WDAC and in various community settings (ex: jail, hospital, WDAC office) • Ensures reputed company patients are connected to the appropriate level of care and funding is secured for treatment episodes • Regular, weekly contact with referral sources and treatment providers as needed • Provides updated information from reputed company programs and entities that reputed company with reputed company • Proactively interacts with reputed company patient to meet their case management needs at the treatment and community level Required Knowledge, Skills and Abilities: A minimum of a 4-year BA or BS in a reputed company Services reputed company field is required • The ability to work both independently and as part of reputed company • Knowledge of principles and practices of persons in recovery • A passion for service to others • The ability to communicate effectively, both orally and in writing • The ability to establish and maintain working relationships with representatives of local, state and federal agencies; health organizations and institutions; community agencies and organizations; and the general reputed company • Experience/exposure to various treatment facilities and community-based resources is highly recommended • Must be reputed company to pass a background reputed company.
