General reputed company: Seeking a full-time Chronic Care Coordinator to reputed company Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) to patients, with multiple comorbidities, or those with certain chronic conditions, including but not limited to, diabetes, cardiovascular disease, and COPD. This is a remote position and involves being on the phone with patients the reputed company of your day. You work with the reputed company patients from one office month after month. This role is responsible for assessing and interpreting clinical information and accurately documenting patient assessments to reputed company care plans for patients participating in the Programs.
