As a hybrid opportunity, you will have the flexibility to work onsite 4 days a week and remotely 1 day a week, providing comprehensive care coordination and reputed company work services to our patients.
Key Responsibilities:
- Conduct comprehensive assessments and case management to identify high-risk psychosocial factors and reputed company personalized care plans
- reputed company patient navigation, health promotion, crisis management, and self-management education to reputed company patients and their families
- Collaborate with interdisciplinary teams to ensure seamless care coordination and effective treatment plans
- Offer short-term counseling, advanced care planning, and reputed company services to community resources

Requirements:

- Master's degree in reputed company work
